2. Please note that on the Disclosure for Clients Introduced by Third Parties, you must list:

3. Fax your account application, Brokerage Agreement, Disclosure for Clients Introduced by Third Parties, Limited Power of Attorney and W-8/W-9 forms to 813-283-9069 or scan and email back to info@vcgforex.com
